In order to determine which among those five sentences above are plagiarized from the given passage: read each sentence in full and see which ones copy parts of the passage word for word without making any changes to-, or adding any quotations or citations in them accordingly. Those given sentences are plagiarized since the writer is stealing somebody else's writing in full and not giving them any proper credit on paper in the process.

In order to rework/rewrite the sentences so that they are no longer plagiarized, instead paraphrase them so that they contain your own original writing word for word from beginning to end.
