Throughout the short story, there are small details that could help understand what is happening. For example, the landlady offered him a cup of tea which "tasted faintly of bitter almonds." This indicates that the teas is in fact poisoned, specifically with cyanide.
It is also revealed that the landlady is a taxidermist, and later she mentions that they are still upstairs together. This is when we can draw on the conclusions that she may have poisoned the other visitors, perhaps just for taxidermy purposes- and the same will probably happen to Billy himself. She even exhibits a specific victim profile, a young-looking male around the age of 17, except for one whom looked about 17.
