Punctuation tends to fall in between quotation marks, except for a few special cases.
If the punctuation is part of what the person is saying, then it falls in quotations. Example: "I want a toy and I want one now!" The speaker is exclaiming this phrase, therefore the exclamation point is part of the quote.
If the punctuation is what the speaker is saying when referring to a quote, then it would fall outside. In this case, the quote itself has no specific punctuation or tone, but the speaker does. For example: Did she really say that she wanted "one trillion dollars"?

Punctuation normally goes inside quotation marks. However, there are exceptions. Question marks and exclamation points sometimes go outside quotation marks. For example, if someone is asking about a specific quote, a question mark would go outside the quotation marks: Did she really say, "I'm not talking to her again"? In this example, the question mark is not a part of the quote, so the question mark is outside the quotation marks.
The same practice could apply when using an exclamation point: I can't believe she said, "I'm not going to the dance anymore"! Again, the quote itself was not exclamatory; rather, the person referencing the quote is showing exclamation in response to what was said.
