It sounds like you have a good grasp of how to set goals, and you seem to actively work towards them for a while. You also realize that you are your own worst enemy when it comes to follow-through. If the way you have been posting your goals works for a time, I would consider continuing to use those methods, but you will need to implement a "fail-safe" once life happens and gets you distracted.
You might try using your phone or other device to incorporate into your goal-setting plans. Maybe make a digital reminder with an alarm that goes off on days you need to be working on something. The alarm might be the extra kick you need to get yourself redirected and back on track.
Another suggestion is to find an "accountability buddy." Along the lines of a gym partner, this person could help you get motivated when you are falling behind and help you refocus. You should be willing to do the same for them. Teamwork makes the dream work!
