I would think that "fascinating" could apply to anything that holds your interest in a pleasurable way. For example, it could apply to a person, a field of study, or even a book or movie. "Fascinating" has a connotation of being enchanting or alluring. A close synonym is "captivating." I think that fascinating already implies "extremely interesting," so you do not need to say "extremely fascinating." Extremely fascinating" is too wordy. "Fascinating" can stand alone.

Extremely fascinating is when something triggers your interest tremendously to the point where you want to explore it in greater detail by choice and for pleasure. When something is extremely fascinating, it also means that you never knew about it before, so it is brand new to you and fresh in your mind.
