Simple answer was the secrecy of the Imperial family and the role Rasputin played. Alexei had hemophilia - the feeders disease. But no one knew outside a circle of close family members. Rasputin capitalized on this and slowly started to take power by manipulating the Czarina and Czar. He claimed he could heal Alexei. He most likely hypnotized him which calmed both mother and son. He was a bad guy who just wanted power.
The Czar, Nicholas II was an ineffectual czar. No leadership skills and it is said he was" like a pillow, bore the impression of whoever sat on him last". He was not deceicive and changed his mind too much, listened to the wrong advisors and was a weak man by nature. Russia was not in safe hands with him at the front, Czarina and Rasputin in the Palace calling the shots and appointing ministers and running Russia into the ground. The Army had enough, the Duma was frustrated, and the people were scared. Politically, Militarily, and Socially the country was a hotbed of revolution and ready to explode.
