I will briefly explain how this is done and then do a couple as examples to get you started.
An active voice sentence has the order of Subject + Verb + Object. To convert that sentence to passive: 1) The object becomes the subject, 2) the verb "be" (am, is, are, was, were) is added to the verb (keeping the verb tense of the sentence), and the action verb is changed to the past participle. The subject of the original sentence becomes a prepositional phrase that begins with the word "by." The prepositional phrase is not always stated, but can be stated if desired.
- The case is referred to a psychiatrist by the resident physician for further diagnosis.
- The ingredients and materials are prepared by you a day before cooking the recipe. (The original sentence is a command with the understood subject "you," which seems a bit strange because it seems to lose the "command" idea. I don't think I've ever seen a command converted to passive voice like this in this type of exercise.
Now here's a sentence that is in passive voice and what it looks like in the active voice.
