First, I recommend that you make the question into a statement as it will be easier to see the parts of the sentence. "He does mean what?" I generally have students identify the verb first. In this sentence the verbs are "does mean." Then look before the verb for the subject, and you find "he" is the subject. Now you are left with one word, "what." "What" is a direct object.
If you are doing sentence diagramming with lines, these three parts of the sentence are all on the main line, one after another (Subject - verb - direct object)
