Oh, welcome to the club, sweetie. And don't be so hard on yourself. The club has a population of about 7 billion :(
Honestly, I think every person on Earth can sympathize with you. Look at all of the crowded gyms in January that are ghost towns by March.
Really, it's about developing habits. One way to do that is change your environment so that it is EASIER to follow your new habits, and HARDER to stick to your old ones. Also, don't try to change everything overnight: focus on one thing at a time. learn to prioritize, too, and do the thing that excites you most - that will probably help you stay motivated.
Remember that you will fail or find fault in yourself every day. Don't use failures as a reason to stop. It takes time to develop new ways of doing things. Humans are built to take the path of least resistance.
Join a group or report to Facebook or Instagram or something that you are doing this new thing. give regular updates with pictures or video, too. It helps to keep you honest if you're doing something publicly.
Put time aside every day or week to work on something that is important to you. tell everyone that you are working / busy at that time so and don't feel guilty about not answering the phone or anything.
Best of luck!

Also, it has nothing to do with determination. You are obviously determined. You just have to clear the path to make it easier to almost mindlessly keep to your schedules.04/08/21
