I think the answer to this question depends a lot on the patient-specific factors. However, I think in general, you would be concerned about the patient having fluid overload if they were 1L positive every day. This likely would signify that the patient is either not adequately clearing fluid (renal impairment) or that they are retaining fluid (heart failure). A thorough workup would need to be obtained and you would want to be looking for things like edema, shortness of breath, anuria, serum creatinine, and electrolytes.
