A 45 year-old woman complaining of breast pain was diagnosed with having HER2-positive breast cancer. The patient underwent lumpectomy followed by a period of chemotherapy including adjuvant treatment. Over the course of their treatment, the patient developed a urinary tract infection that required antimicrobial therapy, which initially consisted of a trimethoprim/sulfametoxazole combination drug. However, the patient during the course of her therapy suffered multiple recurrent infections and was, consequently, enrolled in a clinical trial to test the efficacy of a new mannoside anti-infective agent.


Question: Discuss the mechanisms underpinning current therapeutic strategies for treating HER2-positive breast cancer. In your discussion, you should include details about managing therapeutic resistance and adverse side effects such as cardiotoxicity.

Lumpectomy is a standard part of a treatment plan for most tumor-causing breast cancers. If you reread the question you will see that the patient has already had a lumpectomy, and the question asks about what the next steps are for treating HER2-positive breast cancer specifically. That means that after the lumpectomy the tumor was biopsied and tested via ISC or (rarely) FISH to determine if the cancer cells were HER2 positive. Since they are, the treatment plan needs to be specific to the type of breast cancer. This patient would get better results from targeted drug therapy, which attach to the HER2 protein, it’s a kinase inhibitors and effectively the cell can’t produce that protein anymore, stops replicating, and eventually dies. Targeted drug therapy is often used in conjunction with chemotherapy to be most effective, although if the cancer is caught early chemo may be unnecessary.
