Question #6: I am not sure what exactly they are asking for. There might be activities listed under #5. That's why they are asking for "other activities" in #6. So, I'm not sure what they are referring to.
Question #7 and #8, however, are super easy: the two receptive skills are reading and listening, and the two productive skills are speaking and writing. Hence, the receptive skills you are using when you are conversing is listening, and the productive skill you are using when you are taking notes is writing.
