There can be many societal impacts due to a ketogenic diet. Should one adopt such a diet, it could have profound effects on their physical and mental health, thus, allowing or disallowing them to be contributing members of society which, in turn, would affect their livelihood, their personal relationships with family/friends, or their professional relationships with work colleagues. It largely depends on how that individual's body and mind adapts to the diet.
There are many subversions of the diet too. One could be vegan and follow a ketogenic diet. One could only eat meat and follow a ketogenic diet. One could include both plants and animals and follow a ketogenic diet since a ketogenic diet is one where you eliminate a majority of carbohydrates from your diet to reach ketosis. No doubt that extremes in a ketogenic diet could also have profound impacts on personal relationships with others who may or may not agree with eating all meat or all plants.
in addition, should entire societies adopt an all meat or all plant diet or majority meat or majority plant diet, it would no doubt impact the agricultural environment in that society.
