There are probably large numbers of genetically based disorders that could so result, but I'm betting it's epigenetic, maybe the patient grows flame lilies and has absorbed ridiculous amounts of colchicine, which prevents proper chromosome segregation during mitosis? Or a similar material. Take a patient (and family) history. Sounds like a lethal mutation, if not a chemical.
I'd be curious to know what range of answers your class came up with!
