Mi hermana y yo - my sister and I. This serves as the subject of the sentence. We now know that the subject is first person plural - or "we."
The "se" in "comunicarse" and "besarse" refers to the reflexive function of the language - meaning that these verbs are actions that people do to or with each other.
Try translating the sentence into English first:
Mi hermana y yo nos comunicamos por Instagram - My sister and I communicate (with each other) through/via Instagram.
The "se" suffix is a placeholder in the infinitive verb form. Once you conjugate the verb, you must move this part ("se") before the verb, and then turn it into the proper reflexive pronoun - me, te, se, nos, os, or se (yes, I repeated se - it's used for él/ella as well as ustedes/ellos/ellas). So:
comunicarse -> se comunicar -> nos comunicamos
