When is "al" not interchangeable with "a el"?

This is a clear case where "al" cannot be replaced with "a el":> Al mirarlo, sonrió.Are there any other cases?

When you need a indirect object or direct object you can't omit the el. Al can only be used when neither is needed. Hope that helps.
