The answer is that "muy bien" actually means "very well" as in the response to the question ¿Còmo está Usted? ("How are you?") In English, we often say, "I'm good" when we mean "I'm feeling well, or I'm doing well."
"Bueno" is an adjective that means "good," like a characteristic of something or someone, as in "Es un hombre muy bueno." ("He's a very good man.") or "Es un buen idea." ("It's a good idea." P.S. yes, "idea" is masculine in Spanish!)
