You have been given two points, from which you can compute the slope m = change in y divided by change in x.
Once you have m, you use Point slope form, choosing just one of the above points to use as x1 and y1.
Of course, you must know Point Slope Form--consult your textbook or notes or the Internet (but you should know it).
