Overkill (if all those points are on line one).  Just choose two of those points, find the slope, and then use point-slope form.

 

The same instructions pertain to all of the other lines.

 

This is a SILLY PROBLEM--you should know that "TWO POINTS DETERMINE ANY LINE," and so should the teacher!
