Teacher Recommendation - write the name of a Club Sponsor/Coach and the Club/Organization (that you've had a leadership role in) that would recommend you and validate your application.

Not sure what you're asking here, but assuming you're asking for a letter of recommendation, I would retrospectively look at your past experiences with any Clubs or Sports teams (that you have played both an active role as a member and a leader in). Decide on a person who could best speak for your skills as a person.

Ask them, in person first, if they would be able to do so. If yes, send a formal request to their email, giving at least a month in advance notice. In the email, send your resume, details of the program you are applying for, as well as your application. There are plenty of templates you can find online, make sure you tailor it.
