Mexico City ranks near the top of most polluted areas, after China and India. China would have people wearing mask fasks even before the pandemic, due to the smog. If you don't like face masks, these areas are not where you want to live.
Medical care is less expensive though, partly reflecting lower quality, but also other reasons. Prescription drugs are less expensive. Some elderly Americans charter buses to go to Mexico to buy drugs. You can buy in larger quantities, but you need a Mexican doctor's prescription. That's easy though, they'll prescribe whatever you want and as much as you want. Try to buy drugs without the Mexican prescription and you could end up in jail. An American prescription is not honored.
