y = mx + b is standard form for a linear equation with slope=m and y intercept = b
y=(-1/4)x + b, solve for b by plugging in the point x=8, y=-3
-3 = (-1/4)(8) + b
b = -3 +2 = -1
y= (-1/4)x -1 is standard slope intercept form
there is another standard from
x+4y =-4, call it ax+by = c form, but you probably want the y=-x/4 -1 slope intercept form
