There are several ways to attack this problem; here is one way.
Call the points (u,v) and (p,q)
The slope of the line joining these points is m = (q-v)/(p-u) .... rise/run!
Then the equation is m = (y-v)/(x-u) OR m = (y-q)/(x-p)...this is the point slope form
and if you write out the form without using the m, it is called the 2 point form.
