How to write out dates correctly?

I have a document dated 05/05/2012. What should I say? > * Based on the document from 05 May. * Based on the document from 5th May. * Based on the document from 05 of May.

Thank you for your question. The proper way to write your example in English is May 5, 2012. If your date is within the sentence you must also use a comma after the year. For example: On May 5, 2012, something occurred. If, however, you are only writing month/year, then use no comma: The paper is dated May 2012.
