This question wants you to answer with a descriptive adjective about your friend (amiga).
The key to understanding this question is knowing that es is from the verb SER. We use SER to identify or characterize a person, place, animal, or object (in addition to other uses). If the question were ¿Cómo está tu amiga? it would be asking, How is your friend [doing/feeling]? However, since we have ¿Cómo es tu amiga? we are instead being asked, What is your friend like [as a person]? This could mean physically or in terms of personality.
An example of an appropriate answer is: Mi amiga es alta, pelirroja y muy inteligente. My friend is tall, red-headed, and very intelligent.
