The zeros are at x=-4, x=1 and x=1 again. Write it in factored form:
y = (x+4)(x-1)(x-1)
Note that if x=-4, then the binomial (x+4)=(-4+4)=0. When x=1, the the binomials (x-1)=(1-1)=0. Multiply the factored form out and combine like-terms to put it into standard form.
