y-5= -4(x+1) is the perp line through the given point (-1, 5)
in point slope form
or
y =-4x +1 in slope intercept form
or
4x +y = 1 in standard form
For y = (1/4)x + 2, divide -1 by 1/4 (or 0.25) to obtain -4 which will be the slope of
the line sought.
Then build the equation of the line as y = -4x + b where b will be the y-value of the intersection
point between y = -4x + b and the y-axis.
For (x,y) = (-1,5), y = -4x + b would turn into 5 = -4(-1) + b which forces b to a value of 1.
The required equation is then y = -4x + 1 or 4x + y = 1.
