Start with two point form:

(y - 30.20) / (x - 6) = (54.50 - 30.20) / (12 - 6)

(y - 30.20) / (x - 6) = 24.30 / 6

(y - 30.20) / (x - 6) = 4.05

y - 30.20 = 4.05(x - 6)

y - 30.40 = 4.05x - 24.30

y = 4.05x + 6.10

So the start-up fee is $6.10

An additional 6 hour cost an additional $24.30, That is $4.05 per hour.

Use slope-intercept form

y = 4.05x + b

30.20 = (4.05)(6) + b

30.20 = 24.30 + b

6.10 = b

(12, 54.50) could have been used instead.
