If the question is asking for the equation for the total cost for month, you would solve the problem as follows.
The Cost for Subscription per month is $50
The rental fee is "4.5% of the cost of the equipment"
in math you can translate the word "of" to "times"
SO 4.5% times (cost of equipment)
= 4.5% x ($1000)
= (4.5%/100) x $1000 (change the % into a decimal by dividing by 100)
= $45 is the cost per month for the equipment.
The total cost would be the cost of the subscription plus the cost of the rental fee
Total Cost = $50 + $45 = $95/month
