The slope of the perpendicular is 1/4...the negative reciprocal of the slope of the given line.
The point slope form is 1/4=(y-3)/(x+8).
You should be able to manipulate this into slope-intercept form.

The point slope form you are accustomed to see would be
(1/4)(x+8)=y-3
Then the slope-intercept form is:
y=(1/4)x+5
