if by "y+1 = (4/5)x=4
you meant
y+1 = (4/5)(x-4)
then
multiply by 5 to eliminate the fraction,
and collect like terms the constant terms
5y+5 =4x-16
put x and y terms on the same side, constant term alone on the other side
4x-5y =5+16
4x-5y = 21 is in standard form: ax+by= c where a=4, b=5, c=21
but if you meant, with +4 instead of -4
y+1 = (4/5)(x+4)
then
5y+5 = 4x+16
4x-5y= 5-16
4x-5y = -11 in standard form
the +, - and = keys often get confused if your fingers hit the wrong key
