THEN LEFT THE WORLD BEHIND IN GRACE, - more suitable when used in referece of someones death in peace.
THEN GRACEFULLY LEFT THE WORLD BEHIND,- can be used when someone had left a place or a way of life behind.
IN BOTH SENTENCES SUBJECT IS MISSING , THE CORRECTNESS OF THE SENTENCE COULD BEFOUND BY READING THE SUPPORTIVE SENTENCE AND THE DEEP MEANNG THAT THE ONE IS TRYING TO CONVEY.

'Then gracefully left the world behind" is part of a sentence, but it it missing a subject. Or, you could place it in within a sentence. For instance:
Then, she gracefully left the world behind.
OR
She picked up her coat, then gracefully left the world behind.

Neither of these are complete sentences--did you mean phrases/clauses? Or is the "then" supposed to be "they"? Regardless, though, the two kind of mean different things. To leave gracefully would be like walking out of a room in a graceful (delicate, polite, smooth, etc.) manner. To leave something in grace could be like you are leaving something as an act of grace (forgiveness, goodness, kindness, etc.). So the answer to your question depends on what it is you are trying to say.
