"4 more than" is 4+
"5 times a number" is 5x
"plus two times that number" is ambiguous. "that number" may means x or 5x or 4+5x
"that number" may means something different from "a number"
"increased by 40" is +40, "that number +40" but "that number" is unclear
if "that number" refers to 4+5x then the entire expression is
4+ 5x + 2(4+5x)+ 40 which equals 52 + 15x
or
possibly
4+5x +2(4+5x+40) which equals 92+15x
"that" and "a" probably refer to different numbers, maybe let y= "that number"
4+5x +2(y+40) = 84 +5x +2y or
4+5x +2y + 40 = 44 + 5x + 2y
