Point Slope Form is y-y1 = m(x - x1) where (x1,y1) is a point on the line.
So,
y + 5 = 4(x + 2) would be the equation of a line parallel through the point (-2,-5). (parallel lines have the same slope)
If needed in slope-intercept form, isolate the variable "y". (y = mx + b)
