The Chernobyl nuclear was particularly disastrous because Soviet authorities ranging from people working in the powerplant to the ambassadors initially refused to acknowledge it and then were slow to contain it. Additionally, after the fact, the death toll officially recorded by the Soviet Union was shockingly low, despite the fact that it is believed to have caused thousands of premature cancer deaths. To this day, Chernobyl is still extremely radioactive and will continue to be so for another 20,000 years approximately. The answer is D.
