I am not sure about just giving the answer, so I'll try to make the solution more difficult than it really is. Ok? 

The point (0,8) is on the y.axis, so the y intercept is 8;

The slope is rise over run;  let's see.... starting from (0,8), we run over 2 from the y axis and then go up to 12.

So the run is 2-0 = 2, and the rise is 12-8 = 4.  Slope = rise/run = 4/2 = 2.  The slope is 2.

b = 8, m = slope, 2.

So the equation is y = mx + b.

I guess I better let you finish from there.
