Point slope form is y - y1 = m(x - x1) where m is slope and (x1,y1) is a point on the line.
Givem slope = 4 and passing through (-3,9), then m = 4 and (x1,y1) = (-3,9)
so y - 9 = 4(x--3) --> y - 9 = 4(x+3)
Slope intercept form is y = mx + b, where b is y intercept
start with above equation and solve for y:
y - 9 = 4x + 12
y = 4x + 21
