Let (h,k) be the point that the line goes through, with slope = m
then the equation of the line in point slope form is
(y-k)/(x-h) = m = slope, (h,k) is the point
y-k = m(x-h)
if you want the equation in slope intercept form
then
y = mx -mh+k where mh+k = the y intercept
