the equation y= (1/2)x +6 is the standard form for a line in slope intercept form with slope = m = 1/2 = the coefficient of the x term, when the equation has solved for y. 6 is the y intercept which reflects the y coordinate of the point (0,6) where the line intersects the y axis
y=mx +b is the general form of the equation in slope intercept form
y-h=m(x-k) is the general form for the equation in point slope form where the point is (k,h) Here the only point suggested is (0,6) the y intercept, so that looks like
y-6 = m(x-0) in point slope form
