just plug those values into the slope point equation
y-h = m(x-k) where m=the slope = -1/2, (h,k) = any point on the line, such as (4,1)
y-1 = (-1/2)(x-4)
simplify if you want, multiply by 2
2y-2 = -x +4
x+2y = 6
just to check further, plug (4,1) into x+2y = 6
4 + 2(1) = 4 + 2 = 6
