Point slope form is y - y1 = m(x - x1).
Calculate m, the slope, by rise/run = (2 - 1)/(2 - 0) = 1/2
Then you could plug in either point into the poin slope form equation 2 lines up, and obtain
y - 1 = 1/2(x - 0) which simplifies to y - 1 = (1/2)x or, using the other point,
y - 2 = 1/2(x - 2)
Both of the equations can be verified by plugging x and y values of both points into both equations
