As stated in the comments, this is ambiguous. Possible interpretations:
f(x) = √(x + h + k)
f(x) = √(x) + h + k
Something like "f(2)" means that you replace x with 2 in the expression. If you are given f(x), then you might be asked solve the equation for x.
