First, find the slope from
m = (y2 - y1)/(x2 - x1)
Plug in
x1 = 3
y1 = -5
x2 = 5
y2 = -1
and get m. For the point slope form of the equation, either of these is correct:
y - y1 = m(x - x1)
y - y2 = m(x - x2)
