I understand and this a common circumstance with designing and printing. If you had done this project as part of an ad agency or design studio, the AE would have reviewed it and also have the client sign off each time changes were made, which puts the responsibility back on the client to check it thoroughly (so do that too). Try to find someone to proof each time before you show client, even if it's a family member. Better to have signed proof, than argue who is responsible. It is hard to tell with your description exactly what changes were made and what you may have missed that client also didnt catch til it was too late. I'd say if you didn't have him sign off, you may have to split it, and ask printing rep for a break on your end $.
It's hard to be a one man band and know it all. But we all make mistakes once, then we learn. Life!
