I wrote two sentences. The first gives a form of equation of a parabola.
It should be clear; consult your textbook on the topic of parabolas and their vertices.
The second sentence says to SUBSTITUTE specific values that your problem gives you,i.e. you have an (x,y) point and you have the vertex (h,k); upon substituting these, you will have one equation with the only unknown being "a". 
Solving for a will enable you to write the equation of your specific parabola.
