The startup fee is $30 and must be paid before you begin.  This means that 30 in the y-intercept.  You pay $30 at 0 months.  Then you have to pay $4 every month after that.  This means that 4 is the slope.  So, after x months, the total amount you have paid, y, is 

 

y = 4x + 30
