How do you write an equation for a graph

A graph that is in my homework that I'm having truble with.

Is the graph a straight line?  If so, at what points does it cross the y-axis and the x-axis?

The parabola's vertex is at (4,40)

With the information you provided in the Comments, we can write
 
y = a(x - h)2 + k
 
Vertex is (h,k)
h = 4
k = 40
 
y = a(x - 4)2 + 40
 
But we need some additional information to find "a", such as the value of y at some x other than 4, or a value of x where it crosses the x-axis.

The vertex po i (4,40) the point where it crosses the y-axis is (0,0) and the point where it crosses the x-axis is (8,0)
