Is this grammatically correct or I am missing some commas?
She continued to dilate; artificial rupture of membranes was to be performed with close monitoring.

Its not so much that you are missing any commas, I would recommend looking at the sentence structure. It would read best such as this:
"She continued to dilate. Artificial rupture of membranes was performed with close monitoring."
Where is is confusing, is that I am not clear, as a reader, whether or not the rupture would be performed if she continued to dilate, or that it was a definitive action that was going to take place.
