I agree with Kenneth, but would add the following. One of the correct ways of joining two independent clauses to make a sentence is to use a comma and a conjunction. Adding a comma where Kenneth indicates is necessary to make the sentence correct. You could also take out the "and" and use a semicolon instead of a comma: "What did I notice that reminds me of mistakes I make; how can I avoid repeating that mistake?"
