Slope=4, passing through (2,5)

Write the point-slope form of the line's equation satisfying the given conditions. Then use the point-slope form of the equation to write the slope intercept from the equation.
The formula is (x,y) is y-y1=m(x-x1)
I need your help to work it out and solve it
and finding the slope intercept

Thats not right the slop is 4 and it passes 2,5

Sorry about that Bob.  Looking back now, it seems I only read part of the question.  You did specify a particular formula to use.  But the slope is indeed 4.
